Cleft Palate:
Cleft palate is a developmental defect with an opening on the roof of the mouth. This defective opening in the palate abnormally connects the nose with the mouth. Hitches of Cleft Palate:

This kitty baby had both lip and palatal cleft. He was planned to have his primary cleft lip repair at 3 months of age. Dr. Sunil Richardson performed the surgery under general anesthesia. Following the success of cheiloplasty, the baby was brought back for Palatoplasty at 10 months of age. They stayed in the hospital for 4 days post-operatively. The baby was under proper care and antibiotics. Since the surgery was done at the right age his recovery was well and good.                   At Richardsons, a patient who has been under our medical supervision for the past 10 years had undergone her primary cleft repair elsewhere and had initially come for fistula closure. The closure and bilateral bone grafting was done. This was eventually followed by the distraction to bring the upper jaw forward. Finally, the nose procedure was done and then orthodontics and fixed partial dentures was installed too. After these series of surgeries, the patient had come back again for the restoration of her lip and few scar removal.

Ear piercing made sterile and easy

Ear piercing is an age-old tradition. Earrings are worn both by women and men.

Ear piercing is safe, performed under professionals with proper aseptic and sterile conditions to avoid infections and promotes rapid healing. Wearing earrings of quality metal is good to prevent infections on later dates.

Ear piercing technique:

With proper asepsis, the skin surface is prepared for piercing.

Local anesthesia is given to numb the area so that no pain is experienced. Sterile needles are taken. Using 18 and 26 gauge needle ear hole is pierced successfully. Small sterile stick soaked in the antibiotic ointment is placed to secure the hole created. Stick is soaked in antibiotic to avoid postoperative infections and to allow the tissues to heal quickly within a weeks time. At times a course of oral antibiotics may be advised.

We had a patient at Richardsons Dental and Craniofacial Hospital with torn earlobe resulted from usage of heavy earrings. Sutures were placed and informed her to report back after 6 weeks to check for healing. Healing was satisfactory and the next week appointment was given for piercing. THE FORMIDABLE CHALLENGE

The nose is the most noticed part of a human face. Cleft nasal deformity is indeed a challenging task for the surgeons as it involves skin, mucosa, cartilage and skeletal structures etc. The appropriate approach regarding the cleft nasal deformity correction is debated tilldate. In 1931 Blair and Brown first described about cleft nasal deformity.

CAUSE

The causes are mainly combination of intrinsic and extrinsic factors such as mesenchymal migration and mechanical stresses.Cleft lip is formedby failure in the fusion of the medial and lateral nasal prominences with the maxillary prominence during intrauterine development. Studies have shown that the medial nasal process remains underdeveloped and fails to fuse with the maxillary process in cleft patients. As embryogenesis continues, the unopposed forces, created by the development of the irregular orbicularis muscle further define the abnormal clinical features of the cleft nose.

CLEFT NASAL DEFORMITY

CLEFT NASAL RHINOPLASTY

• Primary
• Intermediate
• Secondary

The aim to perform primary rhinoplasty in both the unilateral and bilateral deformity is to provide better contour, symmetry, and projection of the nasal tip by releasing, undermining, and repositioning tissues and cartilaginous structures. This is performed along with the cleft lip repair to restore the nasal shape and enabling a symmetric nasal growth.

Intermediate rhinoplasty is performed at the pre-school age to achieve a better symmetrical nasal tip and to correct the nasal valve obstruction. Although it is not indicated for every patients. The major goal of surgery is to correct the abnormal position of the cleft-side cartilage so that the nasal growth will not exacerbate the cleft nasal deformity. 

Secondary rhinoplasty is done after complete nasal growth. At the age of 14-16 mostly surgeons go for open approach with cartilage grafts for better supporting of the nose along with improved tip definition. The deformed cleft nose is complicated by the effects of anatomic growth and surgical scarring.
